  1. A written request including the full names of both spouses, approximate date of marriage, and your contact information
  2. A photocopy of your valid government-issued photo ID
  3. A check or money order for $2.50 per copy, payable to the Clerk of the Circuit Court
  4. A self-addressed stamped envelope
